Rates Individual Rates : $150 - $175 per 45-60 minute session Group Therapy Rate : $60 per 90 minute session Reduced Fee (Uninsured) Sliding scale fee available on a limited basis. |
Insurance Services may be covered in full or in part by your health insurance or employee benefit plan. Our office will verify coverage and benefits and obtain the necessary authorizations and patient responsibility (co-payment). Our office submits all insurance billing to the insurance company for In-Network. As long as your insurance company pays the claim, you will only be responsible for paying the co-payment. If, for any reason, the insurance company does not pay a claim, you will be responsible for the full fee. Insurance Plans - In-Network Horizon BCBS and out of state BCBS plans accepted through BlueCard Cigna Amerihealth Qualcare Insurance Plans - Out-of-Network: Payment is made by you to the provider. Our office will provide you with a completed and signed claim form (CMS1500) that you can submit to your insurance company for reimbursement to you. |
